With your physician's referral, you can continue to receive skilled nursing care in the comfort of your own home. Oconee Medical Center's Home Health serves those who are acutely ill, are temporarily disabled, or require long-term care.

Home Health Services include:

  • Wound care
  • Disease management
  • Routine home care services
  • 24-hour on-call nurse access
  • Assistance with daily living activities
  • Rehabilitation support and care
  • Social services

How to Get Started

The admission process begins with a phone call from the patient, a family member, physician, or other healthcare professional. Usually, during the call, we will arrange a time for an Home Health staff member to visit with the patient (and family, if appropriate) to explain how our services can meet your needs - right in your home.

Home Health accepts Medicaid, Medicare, private insurance, and private payment for services.

Making a Transition When the Time Is Right

Oconee Medical Center's Bridge Program, a collaborative effort of Home Health and Hospice of the Foothills, offers patients an integrated approach to care in which experienced hospice nurses and social workers join with home health staff to provide care. One of the program's goals is to provide continuity of care and ongoing support when and if the physician and patient decide hospice care is appropriate.

Patients who may benefit from the Bridge Program include:

  • Chronically ill patients who are not yet emotionally ready for hospice care
  • Cancer patients who have chosen to continue receiving aggressive treatment
  • Patients with respiratory disease who elect intermittent hospitalization with ventilator support
  • Patients with unresolved problems with pain
  • Patients whose symptoms require frequent hospital admission or ER visits for shortness of breath and associated anxiety, swelling, or confusion
